To streamline our process, please login to your ShulCloud account before filling out this form. Parents who are signing up for teens can select a child who already exists in your account. Thank you for filling out this form for every individual teen volunteer participating in the Run. Parents signing up siblings will need to complete the form multiple times.

Midnight Run volunteers can be in 8th-12th grade.

I give permission for my teen to participate in the Midnight Run with Temple Israel's Waxman Youth House. Teens will be driven by parent volunteers and arrive back in Great Neck after midnight.
I give Temple Israel permission to use photos of my teen in its publicity such as in the Voice, on the TIGN website, in local newspapers, etc.
I have explained the behavioral expectations of participation in this volunteer opportunity to my teen.
